 Please Note: The Claims Office will process your claim, except for your per 
                          occurrence Deductible.  Payment of the Deductible
                          and Co-insurance is your responsibility and you will need to pay this 
                          amount directly to the doctor.
 
 VISIT Policy Numbers begin with VTM followed by 
                          your policy number.  If your policy number does 
                          not begin with VTM, you may have a policy with one of 
                          our other carriers.  Before sending in your 
                          claim, please contact our office to be sure your claim 
                          is being directed to the correct Claim's Office.

							How long will it 
                          take to process my claims?Claims may take 
                          up to 30 days to process. Please submit all claims in 
                          a timely fashion and complete all required forms in 
                          order to avoid delays.  A Claim form is required 
                          for All Claims including prescription drugs.  The 
                          Claims Office will not process your claim without a 
                          form.
